Background
Insiza Godlwayo AIDS Council (IGAC) is implementing the Pathways OVC Project in Insiza and Lupane District in Matabeleland South and North Provinces. Pathways is funded by the United States Agency for International Development (USAID) through Catholic Relief Services (CRS). The project is targeting the Orphaned and Vulnerable Children and their Care givers through streamlined individual based interventions premised on the project four domains in Healthy, Safe, Stable and Schooled. IGAC is looking for suitably qualified individuals to fill in the above-mentioned posts.
Position Summary
Assist in the implementation of Pathway project Case Management System in line with the National Case Management System and program expectations. Ensuring that all clients have case-plans and are periodically updated and monitored to guide need-based service provision.
Objective
Case plan development for the newly enrolled project participants (clients); Regular review of project client’s Case plans in line with the National Case Management and Pathways project framework; Updating and monitoring of case plans; Collaborating with key government departments in key case management processes including case intake, assessment, case plan development and case follow ups; Provide ongoing support to beneficiaries through linking them to key services; Participate in community outreach activities including child protection awareness campaigns; Following up/ Monitoring; Conducting re-assessment/ tracking progression on cases that are opened for project participants; Mentoring of community childcare workers; Report Writing; Ensure adherence to Safeguarding policy when dealing with Program beneficiaries; Any other duties as may be delegated by the supervisor.
Qualifications
A Bachelor’s Degree in Social Work /relevant field with a minimum of 1 year Case management Experience; Strong knowledge of OVC programing and Government of Zimbabwe National Case Management; Experience in Case Management processes; Ability to document and follow up on Cases; Team Player; Proficiency in use of Microsoft Office packages; Excellent report writing skills; Good interpersonal skills; Good written and communication skills; Creative problem solving abilities; Demonstrated ability to work in multicultural environment; Should be a team player and able to work with minimum supervision, effective communicator; Flexible to work weekend and after hours and be on call when ever need arise.
